Gout is a type of arthritis caused by the buildup of uric acid crystals in the joints, leading to inflammation and pain. Zurampic is not recommended for patients who have not yet reached target levels of serum uric acid or who have frequent gout flare-ups or tophi. It is also much more expensive than other uricosuric agents such as probenecid. 

Zurampic should be used in patients who are already taking appropriate doses of a xanthine oxidase inhibitor such as allopurinol. The dosage of Zurampic is 200 mg once daily, taken with food and water at the same time as the XOI.

Zurampic has been shown to be an effective and safe treatment option for gout patients who have not achieved target levels of serum uric acid. However, it can cause acute renal failure if taken without a xanthine oxidase inhibitor, at a higher dose than recommended, or in patients with severe renal impairment. Zurampic should not be initiated in patients with an estimated creatinine clearance (eCLcr) of less than 45 mL/min, and more frequent renal function monitoring is recommended in patients with an eCLcr below 60 mL/min.
